															Faith Foundation Outreach Inc operate a part time clinic so our service area is limited. Patients who meet our qualifications must then fill out an application and receive an appointment.

Medical care is offered on the first Saturday of every month. Time: 10:00am – 1:00pm. We provide preventive care and treat medical conditions related to high blood pressure, high cholesterol, non-insulin diabetics, asthma, thyroid issues. We are unable to manage conditions such as common colds, insulin dependent diabetes, cancer, orthopedic concerns, and heart related problems.
How do I qualify?
The following criteria must be met in order to be seen as a patient:
- Must have no insurance, Medicaid or Medicare
- Have a condition that we treat at Faith Foundation Outreach Inc.
Patients that qualify based on the above criteria must then fill out an application and call for an appointment. We DO NOT ACCEPT WALK INS.

I. Operate an affordable community-based clinic
We saw a need for a clinic that would minister to the health and physical needs of those in our community who were of low income, had no insurance, and had little or no hope of receiving the health care they needed for their families.
To achieve this objective, we will operate a community clinic that offers non-emergency primary care, which includes medical care. This will enable us to have greater impact on the quality of physical health care of the citizens of Gwinnett County and neighboring/surrounding counties. We will engage volunteers from many community organizations.
To support organizations, programs and initiatives that demonstrate potential for improving the health of our community such as preventive health screenings
II. Provide Community Health and Wellness Programs/Projects that target the following areas:
Prevention or early detection of diseases
– Provide Bi-annual Community-based Health and Wellness Fair open to the public offering free health, vision screenings such as blood pressure check, blood glucose check, cholesterol check, and more.
General Medical Services:
- Annual physical exam & wellness check
- Acute illness management
- Chronic disease management
- Lab/Blood Tests
On-site lab drawing service provided at a very low fraction of the cost elsewhere. In order to make the most of a chronic care patient’s initial visit to the Faith Foundation Outreach Clinic and to avoid subsequent visits where a patient will incur additional costs, a panel of lab tests are performed. The blood tests include complete blood cell counts, comprehensive metabolic panel with kidney & liver functions, thyroid function, lipid/cholesterol panel etc. A licensed practitioner will interpret and review with patients the final results of all lab work done at our facility.
- Medication Assistance Program
Patient Assistance Program connects patients with pharmaceutical companies that offer medications to uninsured and underinsured at little to no cost. Through a partnership with the Faith Foundation Outreach Clinic, the patient must work with a member of our team to fill out the appropriate forms to get medication at little to no cost. Some forms will be required to be filled out by patient. Patient provided requirements vary depending on the pharmaceutical company.
